If your Access 2 card has expired please fill out the Renewal application form.The expiry date reads month/day/year.

If you have lost your card or had it stolen there is no need to reapply. A replacement card cost $20.00 and will maintain its original expiry date. You can pay online for a replacement card or send a cheque or money order made out to:

Please attach a note to the cheque indicating whose card was lost and any other important information such as a change in address. A change in name on the card is considered a replacement card and will also cost $20.
